MVR / State Driving Record
Your Motor Vehicle Record, often called an MVR or driving record, usually comes from your state driver licensing agency or DMV. Carriers and insurance companies may request this during qualification or review. Each state has its own process, fee, and record options.
Suggested action: Check your state DMV or driver licensing agency website and request the type of driving record needed for your situation.
PSP Report
The FMCSA Pre-Employment Screening Program, or PSP, is a federal report that may include certain crash and roadside inspection history. Carriers may request driver consent to access PSP during screening. Drivers may also request their own PSP report directly through the official PSP website.
FMCSA Drug & Alcohol Clearinghouse Record
CDL drivers can register with the FMCSA Drug and Alcohol Clearinghouse to view their Clearinghouse record and respond to employer consent requests. Owner-operators operating under their own USDOT number may need to register as both employer and driver.
Previous Employer Safety Performance History
When a driver applies with a motor carrier, the carrier may contact previous DOT-regulated employers for safety performance history. Drivers may be able to request a copy, ask for corrections, or provide a rebuttal if they believe information is inaccurate.
Suggested action: Ask the prospective carrier how to request a copy of previous employer safety performance history information they received.
Background Screening / DAC-Type Reports
Some carriers use third-party background or employment screening companies. These may include companies such as TenStreet, HireRight/DAC, Driver iQ, Foley, SambaSafety, SuperVision, or other providers. If a report is used in a decision, the driver may have rights to receive a copy and dispute inaccurate or incomplete information.
Suggested action: Ask the carrier which reporting company was used and how to request or dispute a copy of the report.
Insurance / Claims Reports
Insurance companies may use reports when reviewing coverage, pricing, claims, or eligibility. The report source may vary. If an insurance decision is based on a consumer report, the driver may be able to request a copy from the reporting company listed in the notice.
Suggested action: Ask the insurance agent or insurance company what report was used and which reporting company provided it.
Credit Report, if Requested
Some situations may involve credit or financial screening, especially for financing, lease-purchase, business credit, or certain insurance reviews. Drivers can request free credit reports from the official annual credit report website.

Question 2: If any report affects my approval, can you please provide the required notice, the name of the reporting company, and instructions to request or dispute a copy?
